Kindergarten Enrollment Requirements (2009/20010)

STUDENT MUST BE 5 YEARS OLD ON OR BEFORE OCTOBER 1, 2009

1.         Only a parent or legal guardian may enroll a student

2.         Student must be present to enroll

3.         Two (2) proofs of residency are required

                 IF YOU OWN A HOME:

• Deed or current Metuchen property tax bill
• Current utility bill (gas, electric, phone or water) with your name and

                                       address printed on the document

                IF YOU LEASE A RESIDENCE:

• Lease (current and updated)
• Current utility bill (gas, electric, phone or water) with your name and

                                       address printed on the document

                IF YOU ARE A NON-RESIDENT OF METUCHEN AND LIVE WITH                                                   SOMEONE WHO IS A RESIDENT:                     

§  Non-resident (you) must complete:

                                       Residency Determination Certificate which MUST BE NOTARIZED

§  Metuchen resident with whom you are living must provide:

                                      Deed, current tax bill or lease for their residence

                                      Current utility bill (gas, electric, phone or water) with their name and

                                        address printed on it

                                      Completed “Residency Determination Certificate” which

MUST BE  NOTARIZED

4.         Other documents and information that must be presented at registration:

§  Original birth certificate (with seal) and/or passport

§  Immunization records

§  Physical exam (no earlier than September 2008)

§  Completed Moss School registration packet

§  Legal documents (if applicable) for:

                                                separation        guardianship

                                                divorce                        adoption

                                                custody           foster care

5.         IF APPLICABLE:  Special Services records

            (Child Study Team evaluations and reports, IEP’s, etc)
